Prescription Refill Request Form

Please allow at least 24 hours for order processing. **Requests for controlled substances and herbal medications may take up to 3 business days to be processed**

You will receive a text or email when your order when it is ready to be picked up. If you do not received this notification in a timely manner, please call the office to check on it's status.

Note: Some prescriptions will require an examination of your pet prior to re-filling.
